Laser eye surgery is a medical procedure consisting of the use of laser for reshaping the surface of the cornea. It is used to improve shortsightedness, long sightedness or astigmatism. The first information on laser eye surgery dates back from over twenty years ago, and the FDA approved excimer laser eye surgery in 1995.

During laser eye surgery, an excimer laser, controlled by a computer is used to remove tiny amounts of corneal tissue. The aim of the procedure is to restore normal vision, eliminating the need for glasses or contacts. Although laser surgery has risen in popularity in the past few years, only about 20% of ophthalmologists have been trained to use it.

In one type of laser eye surgery, the outer layer of the cornea, also called epithelium, is removed and an excimer laser is used to reshape underneath tissue. This procedure is called photorefractive keratectomy (PRK).

In a newer procedure, a microkeratome is used to cut a flap in the corneal tissue, which is then lifted and an excimer laser is used to reshape underlying tissue. In the end, the flap is put back to its place. This type of laser eye surgery is called laser-assisted in situ keratomileusis (LASIK). Nowadays, it is more commonly used than PRK.

What Happens During Laser Eye Surgery Procedure

The surgery should take less than 30 minutes. You will lie on your back, reclining in a chair, or on a table. You may be given a sedative before the procedure, to help you relax.

Local anesthetic eye drops are used before the surgery. The procedure takes a few minutes per eye. The actual time when the laser is used for reshaping the corneal tissue is generally less than a minute. An instrument called a lid speculum will be used to hold your eyelids open. A ring is placed on your eye, and the cutting instrument is attached to the ring. A microkeratome, the cutting instrument, is used to cut a flap in your cornea. A laser is positioned over your eye, and you will be asked to stare at the light. This light is to help you keep your eye fixed in one spot when the laser comes on.

Once your eye is in the correct position, the doctor will turn the laser on. Pulses of laser energy vaporize the corneal tissue, and then the flap is placed back into position

You will need to wear an eye shield for protection after the procedure. This will prevent you from rubbing your eyes, and protect your eye from being accidentally hit or poked until the flap has healed.

After laser eye surgery, you should expect some minor discomfort. However, you should be able to see, although not very clearly. You won’t be able to drive on your own, so you need to have someone drive you home or take a cab. You should see your doctor within the first 24 to 48 hours after surgery, and then at regular intervals for about six months. You will have to wait for up to two weeks before using make-up, creams or lotions, to prevent infection or irritation. You should also avoid swimming and hot tubs for about 6 weeks.

Although you should see well the day after surgery, your vision will continue to improve over the next several months. As the eye heals, your vision will continue to improve. It is important to keep all your follow-up appointments with your doctor, to make sure that everything is going according to plan.

For these patients, customized laser eye surgery may provide a better quality of vision by measuring and addressing all of these aberrations as part of the treatment plan.

During LASIK surgery, a small incision is made in the cornea to create a flap. Then, a laser beam works within the cornea to reshape it for vision correction. After the reshaping, the flap is put back into place and serves as a sort of natural bandage.

Will I Need Reading Glasses After the Treatment?

Generally, patients under 40 years of age read well without glasses following the treatment. Patients over 40 may need magnification for reading fine print.

Presbyopia is the term that refers to the natural weakening of the focusing muscles that occurs in our early to mid 40s, causing us to need the magnification that reading glasses provide. The LASIK treatment does not correct or prevent presbyopia. We also will be glad to discuss another option called monovision.

What are some of the possible complications of Lasik eye surgery?

Undercorrection – this occurs when the expected vision correction falls short of the desired outcome. This occurs more commonly with patients who have a high degree of nearsightedness (only objects close up are clear), farsightedness (only objects far away are clear) or astigmatism (images both far and near are distorted). Why? There is more laser corneal sculpting that needs to occur with patients having higher degrees of vision imperfection. When choosing your Lasik eye surgeon, it’s important to ask them what percentage of their patients need retreatment for undercorrection. This should be something that they are willing to freely discuss with you. If not, walk away! This is not to be confused with a planned slight undercorrection for nearsighted patients over forty years old which aids their reading vision. But this is something that you and your Lasik eye surgeon would have discussed before your surgery.

Overcorrection – this complication occurs less frequently than undercorrection and results when the amount of correction (corneal laser sculpting) exceeds what is planned. Slight overcorrection can be temporary and may resolve itself in the first month following Lasik eye surgery. Patients can manage slight overcorrections by wearing glasses until their vision resolves. Some patients with overcorrection may need additional Lasik eye surgery 3 to 6 months following their first surgery.

Dry Eye – many Lasik eye surgery patients may experience the feeling of ‘grittiness’ in their eyes following surgery. This condition usually resolves itself in 3 to 6 months and may be helped by using lubricating eye drops. Patients using birth control pills and patients going through menopause may experience this condition more often. If ‘dry eye’ continues beyond 6 months, your Lasik eye surgeon may recommend blocking your tear ducts with tiny silicon plugs to prevent tears from draining away too quickly.

Corneal abrasion – a small percentage of Lasik eye surgery patients may develop a small corneal abrasion (scrape) caused by the microkeratome (instrument used to create corneal flap) used during surgery. This abrasion is generally not serious and will heal quickly. Your Lasik eye surgeon may temporarily place a thin bandage contact lens on your eye to promote healing. While your abrasion is healing, your vision will be blurry.

Night glare – this annoying condition may not affect your vision clarity but patients may see halos or ghosting of images at night during the first month following surgery. Night glare generally improves in 3 months and often disappears within 6 months. Patients with large pupils and more severe vision impairment may be more prone to night glare.

Corneal flap complication – this occurs when the corneal flap is too small, too thin or is an irregular shape. In some cases the corneal flap may shift slightly following surgery if a patient rubs their eyes during the first 6 hours after surgery. If the flap does shift, ‘wrinkles’ can form causing distorted vision. A second procedure may be necessary to ‘smooth out’ the wrinkles and improve vision.

Infection – although this is the most feared complication of Lasik eye surgery patients, it is extremely rare. If your eye is going to become infected, chances are it will happen in the first 72 hours following surgery and will be treated with antibiotic eye drops. For this reason it is very important to avoid eye makeup, hot tubs and swimming pools for at least the first week following Lasik eye surgery.

The risks of Lasik eye surgery are low with an experienced Lasik eye surgeon but you need to be aware of possible complications prior to surgery. Your Lasik eye surgeon should freely discuss all possible complications of Lasik eye surgery prior to surgery. Do everything you can to put your eyes in the best possible hands.

There are a number of methods used to surgically correct eyesight nowadays but the most popular choice is Lasik (Laser-Assisted In Situ Keratomileusis). However before the use of modern technology such as laser beams and computers, the original technique was Radial Keratotomy (RK). This involved an Ophthalmic surgeon with a steady hand and a sharp scalpel to make a series of radial cuts in the cornea (the front ‘window’ of the eye), which when healed, would flatten and therefore reshape the cornea. This would theoretically correct short sightedness. The problems occurred mainly due to the fact that the depth of the incisions could cause weakening and progressive flattening of the cornea, after the procedure. This could lead to a number of defects such as progressively blurring vision and starburst patterns around lights.

With the advent of the laser, a more modern method became the norm, called Photorefractive Keratectomy (PRK). This procedure involved the removal of the outer cells of the cornea (epithelium), and then reshaping the corneal surface with an excimer laser, a procedure known as ablation. The epithelial cells were then allowed to grow back normally. Because epithelial cells on the eye regenerate exceptionally quickly the cornea would generally heal completely within a few days. This technique was certainly more successful than RK but did have its own complications. Side effects such as halos round lights and reduced contrast vision were experienced by almost everyone, along with some corneal ‘hazing’. Although serious complications were rare there was always the risk of infection due to the lack of epithelium protection during the healing phase.

Then in 1990 LASIK surgery was developed by a Dr.Lucio Buratto of Italy and Dr. Ioannis Pallikaris of Greece, and it quickly became popular because of fewer complications and was certainly more accurate. The procedure involves cutting a corneal ‘flap’ with a special high precision blade called a microkeratome. This flap was then folded back with the front surface epithelium still intact. The cornea can then be reshaped with the laser and the flap is laid back down. The epithelial cells that were cut with the microkeratome quickly heal and therefore the outer surface is a nice continuous layer that securely holds the flap in place permanently. Although quite uncommon, some complications such as dry eyes, debris under the flap, ghosting or poor vision to name a few, do occur from time to time.

Another technique that is also used nowadays is LASEK or Laser-Assisted Sub-Epithelial Keratectomy (as opposed to LASIK). It can be used if the cornea is too thin to undergo lasik surgery where the microkeratome cuts deeper than the epithelium. In LASEK, the epithelium is peeled off as a layer to allow laser ablation underneath. During the procedure the epithelial layer is preserved with a special chemical solution and then replaced afterwards, so as to allow natural healing. There is generally more pain and slower visual recovery with this technique.

PRK and LASEK allow the epithelial layer to heal entirely and therefore don’t run the risks of a dislocated corneal flap which can occur due to trauma, even after many years with LASIK. However, the latter is still the most commonly used procedure.

There is still a lot of fear amongst many about the long term effects of laser surgery, but it has been around for quite a while now in its different forms. It seems that more and more people are opting for this surgery, due to a number of reasons whether they be for convenience or cosmetic. With the information technology of the internet, increased knowledge on the subject is much more widespread, so barring any major negative eventualities this trend will almost certainly continue.

What is LASIK?

LASIK is one of the most popular types of laser eye surgery. LASIK stands for Laser-Assisted In Situ Keratomileusis. This procedure can permanently reshape the cornea, which is the clear covering of the front of the eye, and change its focus power using an excimer laser. Millions of patients have been treated using LASIK, using of a wide variety of different lasers. This procedure can be used to correct myopia, hyperopia and astigmatism.

Prior to the surgery:

Before you undergo LASIK surgery you will have to get your eyes examined by a doctor to see if you are a good candidate. As a result of this, your vision will dim and the pressure may cause some discomfort.

During the surgery:

A small device called a microkeratome will be attached to the suction ring; the blade of which will be used to cut a thin flap on your cornea. The flap is created, exposing cornea tissue. After this, both the microkeratome and the suction ring will be removed. The doctor will now lift the flap and fold it back on its hinge, exposing a layer of tissue called the stroma – the middle section of the cornea.

You will be made to focus on a particular spot and the doctor will begin the laser to remove the corneal tissue, according to the measurements taken at the initial examination. The flap is then put back into position and left to heal.

After the surgery:

As no stitches are required, you will just have to wear an eye patch as protection to prevent rubbing, getting poked, or any kind of pressure that might disturb the healing process. It may take up to three to six months for your vision to stabilize after surgery.

Thank you very much for your inquiry about eyelid rejuvenation.

The short answer to your question is yes, eyelid rejuvenation refers to the correction of loose skin, puffiness and dark circles under the eye which makes a person look tired.

Some patients who only have wrinkles and old looking skin can be made to look much better with a Laserlyte peel also called an ActiveFX Laser Peel and Botox. We are the only center in Boston who has experience with this eyelid rejuvenation technique. The Laserlyte peel or Active-FX Peel with the Lumenis Ultrapulse Encore CO2 Lyte is the newest and best technology available.

Most patients however need an eyelid rejuvenation procedure called a blepharoplasty to make the lower eyelid look refreshed and young.

I also use the laser to do this procedure and use a modern technique called a transconjunctival laser blepharoplasty which involves a tiny incision on the inside of the eyelid to correct the aging changes. The benefit is that there is no visible scar on the outside of the eyelid and the natural shape of the eyelid is not changed.

Most other Plastic Surgeons do this operation through a scalpel incision on the outside of the eyelid which leaves a scar and changes the shape of the eyelid.

Finally, to remove dark circles under the eyes we do a procedure called an arcus marginalis release and fat grafting or tear trough implant to plump up the dark circle. This is a new technique and is also done through the inside transconjunctival incision.

LASIK eye surgery involves cutting a flap of the corneal tissue with a hinge being left at one end of the flap. The flap is then folded back to reveal the stroma, the middle section of the cornea. After reshaping the corneal tissue with the aid of a high precision laser, typically the excimer laser, the flap is repositioned so as to remain in place by natural adhesion until it heals completely.

The LASIK procedure entails certain flap complications. Among the post-operative flap complications, a slipped flap is one of the most common. The corneal flap created during the operation may inadvertently detach from the rest of the cornea. It is imperative that the patient goes home and sleeps after surgery so as to let the flap heal, since the chances of flap dislocation are the greatest immediately after the surgery.

Other types of flap complications include folds in flaps and undesired epithelial in-growth. Post-operative folds in flap typically necessitate repositioning of the flap. Flap related problems are relatively common when it comes to LASIK complications. But they rarely lead to a permanent visual acuity loss. Moreover, the occurrence of flap complications decreases with increased surgeon experience. An expert LASIK surgeon, wielding superlative equipment, is far less likely to commit an error during the surgery.

Since LASIK entails the removal of a part of the corneal tissue, the cornea becomes thinner as a result of the procedure. Myopic shifts at extreme altitudes have been attributable to this thinning of the corneal surface. LASIK technology has advanced in leaps and bounds, with much advanced and risk free procedures available today. Owing to this, the chances of flap complications are rather minuscule. Moreover, there isn’t a large body of conclusive evidence so as to assert the chances of any long-term flap complications due to LASIK.

In summary, LASIK is a viable and safe procedure when it comes to correcting a variety of refractive errors. Furthermore, whatever small flap complications may arise can be rectified with ease.

Finding a LASIK surgery that you are confident about will be able to give you more information about the risks of laser eye surgery.

Though LASIK is a safe and effective procedure, with the latest technological equipment at its disposal, it is simply not suitable for everyone. There are several factors that might preclude the possibility of LASIK surgery for a particular patient. One such crucial aspect is pregnancy.

A woman is not suitable for undergoing LASIK surgery if she is pregnant or nursing. In addition, a woman who has conceived of late or is expected to be pregnant within the following six months is advised to postpone surgery. Typically, a nursing mother should defer for at least two months in the wake of the nursing period.

Pregnancy entails significant hormonal changes, which typically cause dramatic fluctuations in a woman’s refractive state. As a result, a preoperative examination conducted during pregnancy might not reveal a truly accurate refractive error. Therefore, the measurements are all askew and so is the determined amount of surgical correction to rectify the refractive error. Consequently, the patient is operated upon based on a false reading and the induced correction is inappropriate. PRK (PhotoRefractive Keratectomy):

After the corrective eye surgery is done the outer layer must be grown back, in order for the eye to be protected again. Due to this, PRK is known for having a large recovery time. There as some advanced methods avaiable today that use a different excimer laser which vaporizes the epthithelium, however it is not widely used.

LASEK (Laser Assisted Sub-Epithelial Keratomileusis)
LASEK is advanced form of PRK(see below) that uses an alcholic solution on the epithelium which softens the surface before it is removed. With this procedure the epithelium can be replaces once the surgery is finished.

Epi-LASEK
Epi-LASEK is similar to LASEK however it uses a type of barrier to preserve the epithelium layer, which is then replaced after the surgery. The reason for this extra step is to try and help cut down post surgery discomfort, and increase the amount of time required for healing.

As you can see their is several different methods of laser eye surgery; These aren’t even all of them! If you think that eye surgery might be right for you, or are interested in obtaining more information it is recommended to consult with your local physician and eye doctor.

I wear eyeglasses because when I was small I used to love reading books, still do but as a result, as my mum says, my eyesight is weak. My family members are lucky because they don’t need any aid for vision. I have been called many names for wearing eyeglasses, including “4 eyes” and “scientist”.

I have tried wearing contact lenses; they are one of the greatest inventions of the 20th century but unfortunately they didn’t work out for me. This is because I have a habit of washing my face and splashing my eyes, during washing 2 or 3 times a day with cold water. I lost quite a few contact lenses as a result. I stopped splashing my eyes with cold water when my mum threatened me that she would put cello tap on my eyes. Still those were not a good choice for me because I suffer from hay fever, and I constantly have to rub my eyes and put eye drops, especially in summer when the pollen count is higher.

Some people are lucky, doesn’t matter whichever eyeglasses they wear they look cool and intelligent. I am not one of those lucky ones I suppose and which my brother reminds me almost everyday. When I first heard about laser eye surgery I thought that my prayers have been answered. Finally a way for me to get rid of my glasses and contact lenses, I thought. Is it?

The word surgery itself is quite scary, but the good news it, it takes less then a minute for the laser procedure. The problem is you have to be the right candidate for it. How can I let go of resentment towards my mother for being thin and lose weight myself?

I wanted to answer this question because, even if weight is not everyone’s issue, resentment and jealousy torture most of us at one time or another.

Jealousy is an indicator that we believe someone else has something that we cannot attain. We may experience jealousy about someone’s looks, their financial status, their popularity/success, or their personal life. But it all comes down to the belief that we can’t have what they have.

Why would we believe this unless we didn’t think we were equally deserving? So jealousy helps us recognize that we are feeling unworthy in some way. And unworthiness is the bottom line issue for almost all of us. The good news is that it’s curable.

To heal it, we have to change the question from, “How do I get what so-and-so has?” to “How do I get that I am worthy?” What I talk about in my book, Enough Is Enough!, is that we won’t necessarily wake up one morning with a belief in our own worthiness. Like any other belief, it takes repetition and practice to “get it.”

So how do you let go of resentment towards your mother for being thin and lose weight yourself? How do you get that you are worthy of having the body you desire?

Before eating or before opportunities to exercise, you ask what I believe is the most healing question you can pose:

“If I knew my worthiness in this moment, what would I do?”

Whether we want to lose weight or experience more love, success, or happiness, I recommend asking yourself this question at least ten times a day every day. You will find that as your sense of self-worth grows, you will suffer less from jealousy and resentment.

LASIK is by far the most prevalent and safest refractive surgery procedure. It has been employed to treat a host of visual anomalies.
